Estimate the exterior height from ground level or deck to bottom of glass:  feet.

Is the defective unit a double hung window (indicate top or bottom sash), casement/awning,
direct set, hinged door, or sliding door?


 

Window/Door Product ID#:  (if possible, try to locate this number which is printed
somewhere on the metal spacer bar between the panes of glass in black ink. 
Note: this number will only appear on Weathershield windows.)



**Note:  If the original order number or the window/door product id number cannot be provided by customer, then a THC representative will have to make a service call to determine the same. 

If the time span exceeds the one-year labor warranty, YOU WILL BE CHARGED
 AT A RATE OF $65 PER HOUR FOR THIS TRIP AS WELL AS FOR THE
LABOR IT TAKES IT FINISH THE JOB. 

This must be paid to our service person/THC representative at completion of work.  Payment may be made in the following forms:  Check, Visa, or Master Card.  (If paying by credit card, customer must call the office with credit card information before the service person leaves the premises.) 

If the time span does not exceed the one-year labor warranty,
YOU WILL NOT BE CHARGED.**

 WINDOW AND DOOR WARRANTY INFORMATION:

Ø      Seal failures and Low E discolorations/failure in the glass are warranted 100% for the first 10 years after date of purchase; year 10 through year 20 the glass can be replaced at 50% off the list price.

Ø      Stress cracks have a 1-year warranty from the purchase date.

Ø      All window/door hardware have a 1-year warranty from the purchase date.

Ø      Wood and wood components have a 1-year warranty from the purchase date against warping or delamination.  However, failure to install these products properly, using the manufacturer’s guidelines, will void this warranty.

Ø      All Labor under the 1-year product warranty is free; after 1 year the customer must pay for installation/replacement, or any other service/labor -including trip charges.

 

Allow 6-8 weeks for product arrival from the time this form is
submitted back to our  Service Department.

 Upon product arrival, our Service Manager Aaron Miller will call
to schedule for commencement of product replacement.
